Dates: 6th September 2025, 8am registration, ready for a 9am start.
            7th September 2025, 8am to 12 noon.

Participant/Parent/Guardian Release

Where a Participant is over the age of eighteen at the time, this form is signed by that Participant. Alternatively, where a Participant is under the age of eighteen at the time, this form is signed by the parent or legal guardian of the Participant on behalf of the Participant.

I acknowledge that I have agreed to participate in the activities of the Sun Soo ITF Taekwon-Do tournament event. Some of these activities involve risk of personal injury. I understand that the Sun Soo ITF Taekwon-Do cannot safeguard against all such injuries and I expressly agree to assume such risk and waive and release Sun Soo ITF Taekwon-Do, it's officers, agents and employees from any claim of liability, including the negligence of any officers for any loss, damage or injury incurred during the program in which I am participating.

I have read and understand the rules and regulations of Sun Soo ITF Taekwon-Do and will comply with all health and safety measures and standards of conduct set by Sun Soo ITF Taekwon-Do.
